Dutch for BIG registration — why healthcare professionals need Programma II

If you trained as a doctor, nurse, dentist or pharmacist outside the Netherlands and want to practise there, the language condition for BIG registration is Dutch at B2 — which in practice means the Staatsexamen NT2 Programma II.

Programma I will not do. BIG registration is a B2 requirement, and Programma I certifies B1 — a full CEFR level lower.

What this means in practice

  • You sit Staatsexamen NT2 Programma II, all four components.
  • Each component passes separately at 500 points, so you can spread them across sittings.
  • Speaking and writing matter disproportionately in clinical work, and they are the components most candidates underestimate.
  • The NT2 diploma does not expire, so passing early in your registration process is safe.

Why speaking is the usual sticking point

Reading and listening are recognition skills and improve steadily with exposure. Speaking is production under time pressure — the NT2 Spreken component gives you 20 to 30 seconds per task, with one two-minute long turn. Clinicians used to switching to English with colleagues often find this the hardest component, and it is the one worth mocking repeatedly.
