- You can take Step 1 after completing at least two years of medical school (typically the pre-clinical years).
- Most students take Step 1 at the end of their second year.
For International Medical Graduates (IMGs)
- You must be enrolled in or a graduate of a medical school listed in the World Directory of Medical Schools (WDOMS) that meets ECFMG requirements.

Age Limits
There is no official age limit for USMLE Step 1. Candidates of any age can apply, provided they meet education and other eligibility criteria. However, some residency programs may have their own age-related preferences, but this is not a USMLE rule.
Nationality and Citizenship
No nationality restrictions. USMLE Step 1 is open to all nationalities. There is no requirement to be a US citizen or permanent resident. However, your eligibility pathway depends on your medical school location:
- US/Canadian students apply via the NBME.
- All other candidates (IMGs) apply through ECFMG.
If you are an Indian medical graduate (MBBS), you are considered an IMG and must go through ECFMG. You do not need a US visa to take Step 1 — many candidates take the exam in India at Prometric centres.
Special Category Rules
Graduates from Non-WDOMS Schools

Re-applicants and Previous Attempts
- There is no limit on the number of times you can take Step 1, but you must wait at least 60 days between attempts.
- You cannot take Step 1 more than four times in a 12-month period.
